Who operates Forms
Forms is a community application for creating and submitting application forms in Root.

For a specific application, the community that publishes the form decides which questions to ask and how to review answers. Contact that community's staff about its application process.
Information Forms may handle
Depending on the form and the Root information made available to the app, Forms may handle:
- Root account identifiers, display names, community identifiers, and role information used by the app.
- Form titles, descriptions, questions, access settings, and publication status.
- Answers entered by applicants, together with submission time and review status.
- Reviewer names, decision notes, and report details when those features are used.
Form owners choose the questions. Applicants should only provide information relevant to the form and should not submit passwords, payment details, or other sensitive information.
How information is used
Forms uses this information to show forms, associate answers with a submission, enforce the settings configured for a form, and let authorized community staff review applications and record decisions.
Each community or form owner is responsible for explaining the purpose and applicable legal basis for the information requested in its forms. That information should be clear before an applicant submits a response.
Storage and access in the current version
The current Forms client stores form configuration, responses, and report records in browser local storage on the device and browser profile where the app is used. This version does not sync those records to a Forms server database, Root-hosted app database, or Cloudflare Pages. Data is not automatically shared between different members' devices.
People who can use the same browser profile and app data may be able to access information kept there. Records can remain until they are removed through the app or the browser's stored site data is cleared. Clearing that data can permanently remove locally stored forms and responses.
If this page is published through Cloudflare Pages, Cloudflare may process technical request information to deliver and protect it; see Cloudflare's privacy information. Root separately processes account and platform information under its Privacy Policy.
If Forms moves its records to server-side storage or changes how submissions are shared, this notice must be updated to describe that change before the new version is used.
Your choices and requests
You can choose what information to enter in optional questions. Required questions are set by the form owner. You may ask the relevant community about an application decision or the information it requested.
For a privacy request concerning Forms, email realsheepy@outlook.com and include the form and community involved. You can also remove locally stored data using the app's available delete controls or your browser's site-data settings.
Forms should only collect information needed for a stated purpose. Do not use forms to request login credentials or payment-card information.
